It contains personal info such as name, phone, special skills, special needs, pet info and emergency contacts.&nbsp; It is shared within the neighborhood only.&nbsp; MYN does not collect personal info.<\/p>\n\n\n\n<p class=\"\"><strong><a href=\"https:\/\/oakmontvillage.com\/wp-content\/uploads\/2024\/11\/5.-Neighbor-Contacts-Summary.pdf\">Neighbor Contacts Summary<\/a><\/strong>&nbsp;\u2013 (attach as backup to your map).&nbsp; This is a summary of house numbers and each resident\u2019s name, phone, pets, and special needs.<\/p>\n\n\n\n<p class=\"\"><strong><a href=\"https:\/\/oakmontvillage.com\/article\/neighbor-buddy-form\/\">Buddy Form<\/a><\/strong>\u00a0\u2013 share your information with a few selected friends or neighbors so they know how to access you and your home in an emergency.<\/p>\n\n\n\n<p class=\"\"><a href=\"https:\/\/oakmontvillage.com\/article\/myn-neighborhood-leader-form\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Leader Form<\/strong><\/a>&nbsp;\u2013 Neighborhood Leader Form<em>&nbsp;\u2013&nbsp;<\/em>neighborhood leaders are to complete and return the Leader Form to MYN &lt;<a href=\"mailto:MYN@OakmontVillage.com\">MYN@OakmontVillage.com<\/a>&gt; or put in the MYN folder in the OVA office.
